The Clinical Liaison has three primary job
Responsibilities:
1.
S/he provides clinical review of patients referred by the member hospitals for home infusion therapy to determine their capabilities to safely receive services at home with NELC.
2.
S/he is responsible for developing and implementing strategies to capture all referrals from the member hospital's system of care (e.
g.
inpatient, ambulatory, etc.
).
3.
S/he is responsible for relationship building with hospital staff for quality coordination of care between hospital and home.

Responsibilities:
Gathers and communicates to NELC's intake department demographic, clinical and reimbursement information on patients referred to NELC.
Develops relationships and seeks opportunities to bring all potential home infusion referrals to NELC.
This may include developing relationships within the hospital, among ambulatory care providers and within the member hospital's care providers (e.
g.
urgent care, SNF, etc.
) Communicates results of insurance verification to hospital discharge planner and/or patient.
Assists NELC reimbursement department in exploring all possible sources of reimbursement.
Reviews needs and abilities of the patient and caregivers.
Participates in developing a patient specific training plan and plan of care.
Participates in patient training when appropriate as dictated by professional licensure/scope Facilitates the discharge or referral process by relaying orders and documentation necessary for discharge or referral to the appropriate NELC departments.
Acts as clinical liaison between NELC and all appropriate departments at the hospital or the hospital's system of care.
This includes, but is not limited to, the medical staff, care coordination/ case management, social services, pharmacy and nursing departments.
Assists in coordinating services with other home health agencies.
Monitors active and pending NELC patient status while hospitalized and provide ongoing communication re:
patient status to appropriate NELC staff.
Maintains an appropriate patient case load as defined by Director of Nursing.
Comprehensively documents all interactions with patient and members of the healthcare team in the patient record and/or electronic progress notes.
Effectively communicates patient requirements and changes in patient status to appropriate members of NELC infusion and reimbursement teams as well as the Home Health Agency (HHA) staff involved in meeting the patient's home health needs.
Participates in daily patient care meetings and case conferences as required.
Participates as a member of the hospital's care management team in hospital rounds, to identify and provide care coordination and guidance to hospital care team, providers and pt.
referred for infusion.
Ensures that confidentiality of patient information is maintained in accordance with state and federal HIPAA Privacy and Security regulations.
Accurate completion and reimbursement determination of therapy specific reimbursement checklists such as Medicare TPN, Medicare ENT, Medicare Cardiac, etc.

Assists in the development and implementation of new programs and educational seminars that will enhance NELC's ability to meet the health care needs of the community served by the hospital.
Works in conjunction with Partnership Development staff (PD) to identify and capture new business Follows NELC policies and procedures in the delivery of direct patient care and in the normal conduction of company business as dictated by professional licensure/scope.
Assures that professional practice complies with state and federal regulations including, but not limited to HIPAA Privacy and Security regulations and NELC's Corporate Compliance Program.
Participates in company Performance Improvement Program via appropriate completion of Occurrence Reports and Customer Complaints, auditing clinical records and offering recommendations for improvements based on trends noted.

Participates in orientation and training of new personnel as well as home health agency staff as needed.
Participates in ongoing professional development via regular attendance at nursing department meetings, branch in-services, membership in local INS chapter and other educational programs pertinent to his/her clinical practice.
Performs other duties / projects as assigned by direct supervisor.
Adheres to NELC's professional appearance standards.
Educational Requirements:
Associate degree or diploma in Nursing required, but BS in Nursing preferred.
Completion of supervised dietetic internship at an accredited program.
Two (2) years intravenous therapy experience in hospital or homecare preferred.
Current BLS Certification preferred.

Knowledge of HIPAA Privacy and Security Regulations preferred.
Knowledge of JCAHO accreditation process and overall JCAHO standards preferred.

Working / practical knowledge of home infusion therapy administration / equipment use preferred.
Ability to communicate effectively with all customers and members of the healthcare team verbally and in writing.
Good interpersonal and organization skills.
Ability to work independently as well as a part of a team required.
Working / practical knowledge of microcomputer systems including patient documentation systems, Microsoft Office (Word, Excel, Outlook).
Dependability in attendance and job performance required.
